Vestibular disease is sometimes called "old dog syndrome," since it's most commonly seen in older dogs, though it can occur at any age. Most mammals get their sense of balance and spatial orientation from a system of organs in the inner ear and connected parts of the brain, together called the vestibular system. When this system is disrupted, a dog's ability to balance is affected.
There are two forms. Peripheral vestibular disease, the more common type, stems from problems affecting the inner ear and nerves, not the brain itself. Central vestibular disease is much less common but considerably more serious, since it originates within the central nervous system. Both forms cause loss of balance and symptoms resembling vertigo or dizziness. Vestibular disease can look alarming, especially the first time you see it, but most cases, particularly the peripheral form, improve quickly with appropriate care.
Peripheral vestibular disease results from irritation of the nerves connecting the inner ear to the brain. Possible causes include:
Ear infections
Chronic or recurrent inner and middle ear infections.
Ear cleaning
Overly aggressive ear cleaning, or ear cleaners used inappropriately with a ruptured eardrum.
Head trauma
Injury to the head can disrupt the vestibular system.
Stroke
Stroke can contribute to peripheral vestibular disease.
Tumors or polyps
Growths affecting the ear or nearby structures can irritate vestibular nerves.
Meningoencephalitis
Inflammation of the brain and surrounding membranes.
Hypothyroidism
An underactive thyroid can be associated with vestibular signs.
Certain medications
Including aminoglycoside antibiotics (amikacin, gentamicin, neomycin, tobramycin) and loop diuretics.
Congenital defects
Present-from-birth defects of the vestibular system.
Idiopathic vestibular disease
In many cases, no specific cause is ever identified. Idiopathic vestibular disease is actually one of the more common presentations in dogs, particularly those over age 9. In younger dogs, middle ear infection is the most frequent identifiable cause; in older dogs, brain tumors become a more likely culprit when a cause is found.
Central vestibular disease
Can result from inflammatory disease, infection, trauma, bleeding in the brain, reduced blood flow, or cancer.
Abnormal posture
Leaning or head tilt, typically toward the affected side.
Unsteady gait
An unsteady, "drunken" gait, worse on the affected side.
Circling
Circling or drifting toward the affected side.
Nystagmus
Involuntary, rhythmic eye flicking (the slower flick points toward the affected side).
Vomiting
Vomiting resembling motion sickness. Dizziness and imbalance can also bring on excessive drooling, sometimes with nausea. When only one ear is affected, circling and head tilt typically point toward that side, and nystagmus may only appear in the eye on that same side.
Congenital form
Typically becomes noticeable before 3 months of age. Breeds associated with this include the German Shepherd, Doberman Pinscher, English Cocker Spaniel, Beagle, and Akita.
In older dogs
Vestibular disease is often mistaken for a stroke by worried owners. Signs can be intense - a complete inability to stand, nausea, and pronounced circling can make basic activities like eating, drinking, or even going outside to relieve themselves very difficult without help. Some dogs need IV fluids and nutritional support during the acute phase.
A physical and neurological exam helps determine whether the disease is peripheral or central. For suspected peripheral disease, your vet will likely examine the ears closely with an otoscope, and X-rays are sometimes used to assist. Bloodwork, cultures, and cytology help rule out other possible causes. Your vet may also recommend a biopsy of any polyps or tumors found.
If central vestibular disease is suspected, MRI or CT imaging is typically recommended, sometimes alongside cerebrospinal fluid analysis, which can help identify inflammatory or infectious causes.
Treatment aims to address the underlying cause while also providing supportive care for the symptoms.
Motion sickness medication can ease nausea and vomiting. Ear infections should be identified and treated promptly with antibiotics - untreated inflammation can progress to a point where it becomes much harder to resolve. If hypothyroidism is the cause, symptoms typically resolve once the thyroid condition is properly managed. If a medication is responsible, stopping it often leads to full resolution, though some residual hearing loss can occasionally remain. Surgical removal of polyps can be curative, while cancerous tumors generally carry a less favorable outlook.
Central vestibular disease generally has a more guarded prognosis than the peripheral form, given the risk of lasting damage to the brainstem. The good news is that most peripheral cases improve quickly once the underlying cause is identified and properly treated.
Rehabilitation therapy can help a dog relearn body awareness and regain stability. Since dizziness can make normal walking difficult or impossible, food and water may need to be brought closer, and some dogs need temporary hand-feeding or help getting outside to eliminate.
There's no treatment that reliably resolves either the congenital form or the age-related idiopathic form on its own. These dogs need supportive nursing care and a safe, confined space during recovery. Puppies with the congenital form often adapt well and become less visibly affected as they mature. In older dogs, idiopathic vestibular disease typically resolves within 7 to 14 days, though a mild head tilt can sometimes persist permanently.
